A 2018 industrial accident resulting in a fatality was investigated in 2023-24 by GES Tech Group (GES) on behalf of the estate of the deceased (plaintiff). GES reviewed extensive site photographs, case files, police reports, OHSA incident reports and inconclusive engineering analyzes prepared by OHSA experts.
Detailed review of key depositions was also an important as part of the investigation. GES determined that the employer modified an elevated sand hopper in a manner that severely compromised structural integrity. The extremely unsafe condition that resulted eventually caused a catastrophic collapse while the worker was under the hopper.
Non-linear FEA modeling and accident reconstruction animations formed a key part of this investigation.
A settlement was reached in favor of the plaintiff’s estate following disclosure of GES expert opinions.
